Short answer
A telemedicine app connects patients and clinicians for video, audio or chat consultations, with booking, payments, e-prescriptions and medical records. Infikey builds telehealth platforms with encrypted video, consent capture, audit logs and market-specific rules for the US, UK, India and UAE. An MVP typically costs USD 30,000–70,000.

Telehealth rules by market
United States
Patient data is protected health information under HIPAA, so every vendor in the video, messaging and hosting chain must sign a Business Associate Agreement. Clinicians generally need to be licensed in the state where the patient is located, and prescribing controlled substances over telehealth follows DEA rules that have changed several times, so check the current position before launch.
United Kingdom
Providers of online consultations in England usually need CQC registration. NHS-facing products are assessed against DTAC, the Data Security and Protection Toolkit and clinical safety standards DCB0129 (for the manufacturer) and DCB0160 (for the deploying organisation). Health data is special category data under UK GDPR.
India
The Telemedicine Practice Guidelines 2020 define who can consult (registered medical practitioners), how patient identity and consent are recorded, and which medicines can be prescribed in different consultation types. ABDM integration lets patients share records using their ABHA ID with consent. The DPDP Act 2023 governs personal data processing.
UAE
Telehealth services need the right licence from DHA (Dubai), DoH (Abu Dhabi) or MOHAP, follow the relevant telehealth standards, and generally keep health data inside the UAE under Federal Law No. 2 of 2019. Integration with NABIDH or Malaffi may be required for licensed facilities.
Telemedicine app development cost
| Scope | Budget (USD) | Budget (INR) |
|---|---|---|
| Video consult module added to an existing clinic or hospital system | 12,000–30,000 | 10–25 lakh |
| Telemedicine MVP: patient and doctor apps, admin, video, payments, e-prescriptions | 30,000–70,000 | 25–55 lakh |
| Multi-speciality platform with pharmacy, labs, subscriptions, insurance, RPM | 70,000–150,000 | 55 lakh–1.25 crore |
Video provider usage, SMS and cloud hosting are running costs on top. Use the app development cost calculator for a quick estimate.

Technology and security
- Video: WebRTC through a provider that signs a BAA where needed (for example Twilio, Vonage or Daily), with encrypted streams, waiting rooms and network quality fallbacks to audio.
- Apps: Flutter or React Native for patient and doctor apps on iOS and Android; React for the web portal.
- Backend: Laravel or NestJS, PostgreSQL, FHIR-ready data model, queues for notifications.
- Security: encrypted records, role-based access, session timeouts, audit logs of every record view, no PHI in push notifications or logs.
- Hosting: HIPAA-eligible or local-region cloud hosting depending on the market.
Delivery plan
- Discovery: specialities, consultation types, payment model, regulations and integrations.
- Clickable prototype tested with a few doctors and patients.
- MVP build in two-week sprints, with video and payments working early.
- Security and clinical review before launch, plus app store submission (health apps get extra review attention).
- Launch with a pilot group of doctors, then scale.
Project blueprint
Video consultation platform for a specialist clinic network
Based on the requirements we hear most often from buyers like this.
A network of specialist clinics wants to offer follow-up consultations by video to patients in smaller towns. Today follow-ups happen over personal WhatsApp calls, with no records, payments or prescriptions captured properly.
What we would build
- Patient app and website for booking, payment and secure video consultations
- Doctor app with schedules, consultation notes and digital prescriptions
- Clinic admin panel for doctor rosters, fees, refunds and reports
- Consent capture, encrypted records and audit logs aligned with data protection rules
- Reminders and follow-up scheduling by SMS, WhatsApp and push
How success is measured
- Follow-up consultations completed online
- No-show rate for video appointments
- Revenue from patients outside the clinic’s city
- Average patient rating per consultation
